About


The health promotion program aims to support practices to improve patient health outcomes by focusing on the social, environmental and physical factors affecting health. There are many useful links and tools in the ‘Resources’ section of this website, covering topics such as Indigenous health, domestic violence, consumer information, cultural diversity, falls prevention, refugees and cancer screening.

How we can help

  • Finding appropriate community resources for patients including those with specific needs  
  • Assisting with cancer prevention and screening resources
  • Increasing awareness about Domestic Violence through on-site training and resources 
  • Assisting with interpreter and other services that patients from Culturally and Linguistically Diverse (CALD) backgrounds may require  
  • Building health literacy by linking the practice population with health information and resources so that patients can take more responsibility for their health decisions
  • Support to identify and refer Indigenous patients to appropriate services.
