About This Program
The South Australian Chronic Disease Action Plan recognises that secondary prevention, early detection and early intervention strategies can reduce the prevalence of chronic disease, delay disease progression and improve the health and wellbeing of the population as a whole. Chronic disease prevention aims to reduce risk at all stages of the disease continuum and at all life stages.
When risk is identified early, appropriate lifestyle-based strategies can have long term positive effects on health and wellbeing. Evidence supports the use of risk assessment tools; practice based screening, risk reduction programs, and the establishment of registers and recall systems for risk factors and chronic disease.

How We Can Help
gpns is committed to supporting and simplifying access to programs and services that promote prevention and early intervention and offers a central point of referral to two chronic disease risk programs, Do it for Life and Reset your Life (view the Program comparisons sheet). Eligibility for both programs is dependent on a risk assessment using the AUSDRISK tool and Aboriginal and Torres Strait Islander, Type 2 Diabetes Risk Evaluation or 45-49 Year Old Health Assessments may be used as a referral pathway.
- Do It for Life is an SA Health initiative directed towards individuals from targeted populations at high-risk of developing a preventable chronic disease. The program supports lifestyle changes related to the SNAPS risk factors for chronic disease and is delivered through Southern Primary Health Centres.
- Reset your Life is a Commonwealth Government initiative for prevention of type 2 diabetes. The program targets 40-49 year olds (15-54 years Aboriginal or Torres Strait islander) and is delivered by gpns and accredited providers at selected sites across southern Adelaide.
Referral to both these programs is via the gpns Chronic Disease Risk Program Referral.

AUSDRISK
The Australian Type 2 Diabetes Risk Assessment Tool was developed by the Baker IDI Heart and Diabetes Institute on behalf of the Australian, State and Territory Governments as part of the COAG initiative to reduce the risk of type 2 diabetes. It is widely used to measure risk of chronic disease.
The Australian Absolute Cardiovascular Risk Calculator
Produced by the National Vascular Disease Prevention Alliance (NVDPA) for the information of health professionals. Single risk factors (like cholesterol level) provide a poor estimate of a patient’s CVD risk. Absolute risk assessment provides a more accurate estimate of overall, individualised CVD risk, thereby allowing the clinician to best tailor pharmaceutical and lifestyle management to the patient
SNAPS
Smoking, poor nutrition, excess alcohol consumption, physical inactivity and stress are recognised as the most important behavioural risk factors that affect the health of the Australian community. The SNAP Guide was developed by RACGP to help GPs to systematically target patients and offer treatment appropriate to their needs. Stress has been added to the SNAP risk factors due to its prevalence and association with the National Health Priority Areas.
